Thank you so much for sharing your story with us. The stories of people living with deafness, tinnitus and hearing loss, and the people who help and support us, are an essential way we communicate how important our charities work is, but we also know it is your story and it is personal to you.

Whenever we would like to use your story we will always check with you first. We have been in touch to explain how and when we would like to use your story, all you need to do is complete this form to let us know if you give us permission to use in the way described.


  • As we are an organisation that deals with real stories from real people, we would like to use your name in stories, photos and films. Please tell us if you consent to RNID identifying you in connection with your story/ photo/ video as follows:

  • Please use format dd/mm/yyyy
  • Please confirm where we may use your information, such as RNID website, social media channels, printed documents, external newspapers and magazines, broadcast media and/ or fundraising materials.
  • We will share final versions with you to make sure you’re happy and if you’d like a copy of anything your story is used in, please let us know and we would be happy to send it to you.

    You have the right to withdraw any of the consents you have given in this form at any time. If you want to access, change, destroy, withdraw consent or change how we use the personal information we hold about you please contact [email protected] and ask for the stories manager. If we support you please ask your support worker.

    Please note we cannot recall materials which have been printed and distributed before you have withdrawn your consent. We will delete posts from our website and social media channels. However, please note once these posts are on our website or social media channels, people can copy and share the posts on websites and social media channels which we do not control. Therefore, we would not be able to delete posts controlled by others.
