Consent to store form

Thank you for agreeing to share your story with us. We find the most impactful way to engage people with our charity’s work is by sharing real stories from real people.

This form allows us to record your consent to hold your personal information and to contact you to ask for your permission to use it in future.

We may wish to use your story, photo or film in lots of ways. For example, on our website, in fundraising appeals, in national newspapers, on social media like Twitter, in reports we write or on television. We will always ask your permission to use your story and will share final edits with you to make sure you’re happy. If you would like a copy of anything your story is used in, please let us know and we would be happy to send it to you.

  • The personal information above will be kept securely within RNID. Access to this personal information will be restricted to those RNID staff and volunteers who need to have access to it.
  • We might want to use your story internally within RNID on printed or digital materials, such as reports or staff presentations. Are you happy for us to do so? Please indicate so here.
  • We will keep your story, photo or film in a safe way on our database for two years. After this time, we will contact you to ask if we can keep using your story. If you are happy for us to re-contact you in the future to renew your consent, please indicate here.
  • How we will hold your information
    We will keep and use your personal information for two years according to your consents, as set out above.

    If we do not ask you to renew your consent before the end of two years, your story, photo and video in which you feature will be securely destroyed.

    You have the right to withdraw any of the consents you have given in this form at any time. If you want to access, change, destroy, withdraw consent or change how we use the personal information we hold about you, please contact [email protected] and ask for the stories manager. If we support you, please ask your support worker.

    Please note we cannot recall materials which have been printed and distributed before you have withdrawn your consent. We will delete posts from our website and social media channels. However, please note once these posts are on our website or social media channels, people can copy and share the posts on websites and social media channels which we do not control. Therefore, we would not be able to delete posts controlled by others.
